Per Evolent Clinical Guideline 044 (Lumbar Spine MRI), MRI is indicated for back pain with ANY of: new or worsening objective neurologic deficits on exam; failure of conservative treatment for a minimum of 6 weeks within the last 6 months; progression/worsening of symptoms during conservative treatment; or abnormal EMG/nerve conduction study indicating lumbar radiculopathy. Also covered for evaluation of neurologic deficits, pre/post-operative evaluation, trauma, known or new compression fracture, pars defect/spondylolisthesis, tumor/metastasis, suspected infection/abscess, and cauda equina syndrome. Imaging in the first ~6 weeks of acute uncomplicated low back pain without red flags or neurologic deficit is not supported.
