Per the Carelon Clinical Appropriateness Guidelines (Imaging of the Spine), MRI of the lumbar spine is medically necessary when EITHER (1) neurologic exam findings suggest lumbar nerve root or cord compression that is new or has not previously been imaged; OR (2) pain or radiculopathy persists after at least 6 weeks of conservative management. Conservative management is defined as physical therapy AND at least one of: anti-inflammatory medication, muscle relaxant, epidural steroid injection, or alternative therapy. Red-flag exceptions allowing imaging without completing conservative care: severe neurologic deficit, suspected cauda equina syndrome, malignancy, fracture, or infection. Prior authorization is not required when the imaging is a component of an inpatient stay or emergency care.
